LS/CMI Assessment Request
Effective October 1, 2019 the Level of Service Inventory - Revised (LSI-R) will no longer be provided by the Council of Accountability Court Judges (CACJ). The Level of Service Case Management Instrument (LS/CMI) replaces the LSI-R. CACJ will provide LS/CMI assessment uses to accountability court programs upon request. If your program needs LS/CMI assessment uses, please complete the request form. Please remember that each certified accountability court is required per statute to administer a risk assessment for each participant to help determine program eligibility.
LS/CMI Training
Each team member performing the LS/CMI assessment must complete training using the LS/CMI user on-demand training provided by Multi Health Systems, Inc. Coordinators should request the training using the LS/CMI User On-Demand Training Request form. After completion of the LS/CMI on-demand training, please submit a copy of the LS/CMI training certificate. Both links are provided below.

- LS/CMI Refresher Training Request - Beginning February 1, 2021 CACJ will provide LS/CMI Refresher training for accountability court team members previously trained to administer the LS/CMI assessment. Please continue to monitor the website for additional details and open registration.
GEARS Account
The GEARS platform is used to execute the LS/CMI. The GEARS platform MUST be set-up by each court before the LS/CMI can be administered. CACJ does not provide quick score forms.
